Latest update – Now a compensation of Rs. 5 lakh will be given in case of death of the family member of the farmer. If the farmer’s son or daughter or daughter-in-law or husband dies in an accident, then the same compensation will be given. This means that all the families of the farmers are now covered. This scheme will benefit 2.5 crore farmer families of Uttar Pradesh.
Mukhyamantri Durghatna Bima Yojana will provide benefits of up to Rs. 5 lakhs to the insured and their family members in the event of an accident.

- Claim Form 1 – In the event of accidental death (before making of Kisan Durghatna Bima Care Card) of Head of Family. It will appear
- Claim Form 2 – In case of disability of head of family due to accident (before making of Kisam Bima Yojana Card). It will appear
- Claim Form 3 – This form has to be filled up for getting first aid (first aid benefits) in non-ineligible hospitals before making insurance card. Admission of the patient to such hospitals is necessary. Maximum profit Rs. Up to 25,000 can be availed. If completed, insurance companies will reimburse this amount after treatment.
- Claim Form 4 – This form has to be filled for treatment in empaneled hospitals before making Bima Care Card. Patient admission to hospitals is necessary.
- Claim Form 5 – Accidental death of the head of the family (after preparation of Kisan Malnutrition Insurance Card).
- Claim Form 6 – In case of disability of the head of the family due to accident (after making the Kisan Insurance Scheme card).

Mukhyamantri Durghatna Bima Yojana – Eligibility to apply
All the people who want to avail this insurance facility, must meet the following eligibility criteria: –
- The head of the family, male or female, falls from 18 to 70 years of age, and
- He / she should be registered as an account holder / co-account holder in Khatauni *, and
- Annual family income from all sources should be less than Rs. 75,000 per year.
* Khatauni is a list of total village land owned by the head of the family / single family using agricultural related legal documents (Khasra).
Benefits of UP Kisan Poverty Insurance Scheme
Candidates can avail the following benefits through this accident insurance scheme: –
- In case of accidental death / disability of the head of the family, the insurance company Rs. Will provide. 5 lakhs.
- In the event of an accident, people will pay Rs. 2.5 lakhs. 1 lakh additional assistance will be provided if required.
- In addition, accidents in the vicinity of the state will also be covered.
- People in all the government can avail free treatment. Listed hospitals and private hospitals with more than 30 beds. About 1540 hospitals will provide free treatment.
- First aid facility Rs. 25,000 patients in all the nearest hospitals (with a minimum of 10 beds).
