- Cerebral palsy refers to a group of non-progressive conditions of a person characterized by abnormal motor control posture that occurs in the period of prenatal, postnatal or infant development, as a result of brain insults or injuries.
- Low vision means that a person with visual function disturbances, even after treatment with standard refractive correction, is able or potentially able to use vision for the planning or execution of a task with the appropriate assistive devices.
- Locomotor disability means disability of bones, joints or muscles leading to substantial restriction of movement of organs or any form of cerebral palsy.
- Leprosy means a person who has been cured of leprosy, but is suffering from-
- Loss of sensation in the hands or feet as well as loss of sensation and paresis in the eye and eye lids but with no manifest deformity.
- Overt deformities and paresis but having enough mobility in their hands and feet enables them to engage in normal economic activity.
- Excessive physical deformity as well as advanced age which prevents him from doing any lucrative business.
- Mental retardation means a state of arrest or incomplete development of a person’s mind, which is particularly characteristic of the subordination of intelligence.
- Mental illness means any mental disorder other than mental retardation.
- Hearing improvement means loss of frequencies of better ear in sixty decibels or more

- Click on the register link to register with the UDID web portal.
- Log in to the website using credentials.
- Click “Apply Online for Disability Certificate”
- Reads the instructions and fills an online application form.
- Upload all required documents.
- Send the application form to the CMO Office / Medical Authority.
Verification Procedure for UIDID Disability Certificate
- First, the CMO office / medical authority will verify your data.
- The CMO office / medical authority will then appoint the specialist concerned for evaluation.
- Expert doctors will assure the disability of PwD and give an opinion on the disability.
- Finally, the Medical Board reviews the case and provides a disability percentage.
- The CMO office produces a disability certificate and a UDID and disability certificate.
