Uttar Pradesh Chief Minister Yogi Adityanath has announced that people can now apply for a birth / age certificate through online mode. People can fill the UP Age Certificate Online Application Form on the official website of the Department of Medical Health and Family Welfare (Dept of Medical Health & Family Welfare). Interested person can apply online for the age certificate in Uttar Pradesh on the official website up-health.in.

The Department of Medical Health and Family Welfare is currently providing 9 online services – Medical Institution, Medical Illness / Fitness Certificate, Disability Certificate, Immunization Certificate, Death Certificate, Payment for Unsuccessful Family Planning, Payment of Medical Reimbursement, Medico-Legal Certificate And Age Certificate (Medical Establishment, Medical Illness / Fitness Certificate, Disability Certificate, Immunization Certificate, Death Certificate, Payment for Unsuccessful Family Planning, Payment of Medical Reimburse ment, Medico-Legal Certificate and Age Certificate). This department provides 9 online services under the Public Interest Guarantee Act. People will no longer need to make unnecessary rounds in the respective offices and can avail all these services at their home.

UP Birth-Age Certificate Online Application & Registration Process

Step 1 

First of all, interested candidates will have to visit the official website of the Department of Medical Health and Family Welfare (Dept of Medical Health & Family Welfare).

On the homepage, click on the link “Issuance Age Certificate” under the ‘Online Service‘ section at number 9.

After this, the UP Age Certificate Registration Form will appear

Here the candidates have to fill all the necessary details and click on the “Register” button. After this, candidates will have to “Verify OTP” to complete the registration process.

Step 5

Next candidates will have to “Login” and open the Citizen Dashboard.

Click on the link “Issuance Age Certificate” to open the age certificate online application form here.

Step 7

Finally, fill the application form and click on the “Save” button to complete the application process.

Step 8

After submitting the application, the concerned authorities will issue the age certificate within the stipulated time period.
